Why might you need a Case Advocate?

Sometimes there are a lot of different people telling you what they think would be best for you

They can be difficult to understand sometimes

It can be confusing

It can be hard to say what you want to do because you may be worried about upsetting people

If you think you or someone you know needs a Case Advocate, you can get a referral form by clicking on the picture.  It is in Microsoft Word format.

 

You can either print it out and complete it by hand or fill it in on a computer, save it and then email it to us.

 

A Case Advocate will talk to you about what you want and help you say this in big meetings.

 

A Case Advocate can help to sort out problems you might have such as:

Making choices about where you live

 

Complaining about things that upset you

 

Speaking up for your rights

 

The case advocate will talk to everyone involved with the problem

But will be there to support you

Remember they are

Your choices

Your decisions

It is your life !
